In order to apply for your Arizona Drivers License, if your under 18 you mush have a parent or legal gaurdian sign a section of the drivers license application. You will need two forms of identification such as a birth certificate and a social security card. You will have to pas the written, vision and road skills tests. I didn't find any information on the application costs but the drivers license fee is based on age and is as follows:

Age 16 - 39 $25.00
Age 40 - 44 $20.00
Age 45 - 49 $15.00
Age 50 and above $10.00
